Service Description

On Stimulant Medication or missing certain items requested by your local provider to be medically cleared. With all the changes in regulations, medical clearance is peace of mind both for patients and providers. We are offering packages to include your EKG reading, Pre paid lab work order cbc, cmp, Thyroid Tsh, blood pressure, heart rate, and letter for your provider that is prescribing your Stimulant treatment. Don't see an available appointment, contact us to see if we can accommodate you for weekend or evening appointment! We try our best to be flexible whenever possible. We can send your Quest labs to any zip code of your choice when you pay your package and setup your in office appt to complete your blood pressure, Ecg, heartrate reading in person. We advise you pay your package first, provide zip code of your local quest you would like to visit for lab work, and schedule appointment within 1-2 weeks to ensure we have your results during your in person visit, where we can provide documentation for your provider as needed for clearance from primary care. If you are insured, I am unable to charge cash for services covered. Insurances I am actively contracted with is United Healthcare, Aetna (pending) and Medicare. Any PPO insurance we can provide a receipt if needed for you to provide for your insurance but do not guarantee they will reimburse.

Cancelation Policy

Cancellations less than 24 hours will not have deposit refunded. No show cancellation will not have deposit refunded.
